Collection, Use and Disclosure of Personal Information
Professionally, we collect, use and disclose personal
information only for the purposes of providing
care and treatment or the administration of that
care, or for other purposes expressly consented
to by the patient. · Please be aware that there
are some limits to confidentiality dictated by
the law. The information you share with your mental
health therapist, will remain confidential within
the Mental Health Program, except under the following
circumstances:a) If you may be a danger to yourself
or others . b) If there is a child abuse issue
involved. c) If you have been/are sexually abused
by a regulated health professional. Information
of this manner must be reported to the College
of which the professional is a member. However,
it will be your decision whether or not your name
is disclosed in this case.· Patient information
is sensitive by nature. Employees and all others
in this office who assist with or provide care
(including students and volunteers) are required
to be aware of and adhere to the protections described
in this policy for the appropriate use and disclosure
of personal information. Professionals within
the Mental Health Program may share information
for the purpose of consultation and/or supervision.
When this occurs, the information remains confidential
within the centre.· Your consent will always be
obtained in order to share information about you/your
assessment and/or treatment with other health
providers involved in your care (e.g. referring
physician). Your consent (oral or written) is
required before we will disclose personal information
to third parties for any purpose other than to
provide care or unless authorized to do so by
law.· You should also be aware that clinic files
are not “privileged” documents. A court of law
could subpoena the records. In such cases, every
effort will be made to satisfy the subpoena with
a letter that could be discussed with you before
sending it to the judge.
